Responsibilities

• Provide resident-centered nursing care.

• Administer medications and treatments as ordered.

• Monitor residents’ conditions and communicate changes to the care team.

• Maintain accurate, timely documentation.

• Work with residents, families, and teammates to support each resident’s care plan.

Qualifications

• Active Ohio LPN license.

• Strong communication and nursing skills.

• A compassionate, dependable approach to resident care.

• Long-term care experience preferred.
