ICK-Assurance Home Health Care Inc. is a Medicare-certified home health agency in Fitchburg, Massachusetts. We’re under new ownership and rebuilding from the ground up. We’re a small agency today, and we’re growing.

Because we’re still growing, our per-visit rates are modest right now. We’d rather tell you that up front. As our patient numbers grow, so will our pay, hours and positions. The nurses who help us build now will be first in line for those opportunities.

The role
You’ll handle our assessment visits, working closely with our RN clinical manager and our LPN team, and choosing visits that fit your schedule.
What you’ll do
- Start-of-care visits, with the comprehensive assessment and OASIS
- Resumption-of-care, recertification, transfer and discharge OASIS visits
- Skilled nursing and medication administration visits as needed
- Draft plans of care for clinical manager review and coordinate with physicians
- Home health aide supervisory visits and aide care plans
- Teach patients and caregivers; report changes in condition to the clinical manager
- Complete documentation in our EMR within agency timelines
Requirements
- Active Massachusetts RN license (or compact license valid in MA)
- Home health OASIS experience required (start of care, resumption of care, recertification, discharge)
- Current CPR/BLS
- Reliable car, valid driver’s license and car insurance
- Comfortable documenting on a tablet or laptop (WellSky/Kinnser experience a plus)
- Strong assessment skills and the ability to work independently in the field
