Licensed Practical Nurse LTC

We are seeking a Licensed Practical Nurse to provide compassionate, skilled care to geriatric residents, residents with dementia, and residents requiring skilled nursing in a long-term care setting. Working within a unit-based care team, this role supports resident well-being through clinical care, care planning, documentation, and collaboration with residents, families, and nursing assistants.

12 Week Contract with the option to extend

Shift: 12 hr day/night

Responsibilities

• Provide resident care for geriatric residents, residents with dementia, and residents requiring skilled nursing.
• Administer medications and monitor vital signs.
• Deliver wound care and other prescribed treatments.
• Contribute to treatment planning and coordinate resident care.
• Complete accurate charting, care planning, and clinical documentation.
• Monitor resident safety and advocate for resident needs.
• Communicate with families and members of the unit-based care team.
• Supervise nursing assistants during assigned shifts.

Qualifications

• 2 years of experience in long-term care nursing.
• Practical Nursing Diploma.
• Active Licensed Practical Nurse license.
• Basic Life Support certification.
• Knowledge of medication administration, long-term care nursing, electronic health records, wound care, patient assessment, and infection control.
• Experience with clinical documentation.
• Strong teamwork and communication skills.
