Primary Care Assistant - LPN, FT, Days

Find your calling at Mercy!Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Minimum Requirements:
Education: Diploma in Practical Nursing
Licensure: Active LPN licensure
