Nurse Practitioner or Physician Assistant: Geriatric Primary Care

Lafayette, Lebanon & Whitestown, IN | Full-Time | Monday-Friday (8h/day)

About the Role

Join My Mobile MD, a growing geriatric practice that brings primary care directly to residents of assisted living, independent living, and memory care communities. Our patients are warm, appreciative, and glad to see you, and the work is built around relationships, wellness, and chronic disease management.

You’ll cover the Lafayette, Lebanon, and Whitestown area, visiting one community each day and each community weekly. This gives you continuity with your patients and a predictable schedule.

What You’ll Do

· Provide primary care and chronic disease management to geriatric residents on site

· Build lasting relationships with residents, families, and community staff

· Work alongside a full-time LPN in each community who serves as both case manager and administrative support

· Practice with a high degree of clinical autonomy, backed by a collaborating physician

Qualifications

Required

· Current NP or PA license in Indiana (or eligibility to obtain it)

· Active DEA registration (or eligibility)

· Board certification (AANP, ANCC, or NCCPA)

· BLS certification

· Reliable transportation to travel between communities

Experience Preferred

· 2+ years as an NP or PA in primary care, internal medicine, family medicine, geriatrics, hospital medicine, or long-term care

· Familiarity with assisted living, skilled nursing, or memory care settings

· Experienced new graduates with strong clinical backgrounds are welcome to apply

Compensation

Our providers start with a competitive base salary of approximately $120,000-$130,000 and move to a production-based model once their visit volume exceeds their base. Our providers regularly earn $170,000+, and several exceed $200,000 annually.

Pay: $120,000 - $200,000+ per year
