Care Manager-RN or MSW, Aging Life Care Professional, CMC, CCM

RN Life Care Advocates seeks and experienced, compassionate, professional to provide individualized care management and advocacy for older adults and individuals with complex health and psychosocial needs.

The Aging Life Care Professional assesses needs, develops personalized care plans, coordinate services, and guides clients and families through healthcare decisions and life transitions. This position requires sound, judgment, initiative, professional written and oral communication, and a commitment to preserving each client's dignity, autonomy, safety, and quality of life.

Key Responsibilities;

• Conduct comprehensive assessment of physical health, cognition, emotional well-being, functional abilities, living environment, support systems, and available resources.
• Develop, implement and update individualized care plans that reflect the clients preferences, goals, risk, and financial resources.
• Coordinate Healthcare appointments, accompany clients when needed and communicate recommendations and follow up needs within the client system.
• Advocate for clients across home, hospital, rehabilitation, Assisted Living, Memory Care and long-term care settings.
• Support safe care transitions through discharge planning, service coordination, and timely follow-up.
• Monitor changes in condition, identify emergent concerns, and communicate promptly with appropriate providers and authorized representatives.
• Assess psychosocial needs, caregiver stress, family dynamics, and barriers to accepting care.
• Educate and support families navigating aging, dementia, chronic illness, long-term care, and end of life decisions.
• Coordinate with caregivers, healthcare providers, community resources, and authorized legal and financial professionals.
• Maintained accurate, timely assessments, care plan updates, daily progress notes, communications, and billable service records.
• Participate in case conference, team meetings, and professional development.
• Reliable transportation

Minimum Qualifications:

• Bachelor's degree in nursing, social work, dermatology, or a closely related health or human service field.
• For RN Candidate, and active, unrestricted Florida, RN license; BSN preferred.
• Microsoft Office applications to complete documentation, communicate with clients and providers, manage schedules, organize information, and support efficient care coordination.

Preferred qualifications

• Care Manager certified (CMC) or Certified Case Manager (CCMcredential
• Minimum of five years of relevant experience working with older adults or individuals with complex care needs,individuals with disablities and behavioral health.
• Demonstrated education or experience in psychosocial assessment, care planning, family communication, and resource coordination.
• Knowledge of aging related conditions, dementia, healthcare systems, and community base services and state and federal entitlement programs.
• Knowledge of Medicare, Medicaid and Long Term care Insurance.
• Strong assessment, organization, documentation and problem-solving skills.
• Ability to work independently collaborating effectively with an interdisciplinary team.
• Experience, supporting families through difficult decisions, conflicting priorities, and resistance to care.
