Commack, NY | $24.00–$25.00/Hour

Phaxis Healthcare is seeking an experienced Mid-Level Supervisor for an exciting opportunity with one of our clients in Commack, NY. This is an excellent opportunity for someone with a strong background supporting individuals with developmental and intellectual disabilities who is ready to step into a leadership role.

Schedule

• Friday & Saturday: 10:00 PM – 6:30 AM and Sunday & Monday: 2:00 PM – 10:30 PM or
• Tuesday-Saturday 2pm-10:30pm.
• 4-day work week (Off Tuesday, Wednesday and Thursday)

Pay

$24.00–$25.00 per hour

What You'll Do

• Supervise and support Direct Support Professionals (DSPs) during assigned shifts
• Ensure individuals receive quality, person-centered care
• Assign staff responsibilities and oversee daily operations
• Monitor staff performance, attendance, and training
• Maintain accurate documentation and ensure regulatory compliance
• Assist with scheduling, appointments, recreational activities, and daily programming
• Help maintain a safe, clean, and organized environment

Qualifications

The ideal candidate will have:

• At least 2 years of experience caring for children or adults with developmental, intellectual, or physical disabilities (Required)
• A valid driver's license (Required)
• High School Diploma or GED
• Previous supervisory or leadership experience is a plus
• Strong communication, organizational, and problem-solving skills
