SSM Health is seeking an experienced Physician Assistant to join our collaborative Complex Care Clinic team. This unique outpatient role focuses on helping patients optimize their health before surgery and supporting successful transitions following hospital discharge, ultimately improving outcomes and reducing readmissions.

What You'll Do:

• Conduct pre-surgical evaluations and determine optimization needs using established clinical pathways

• Collaborate closely with primary care providers and specialists to coordinate patient care

• Evaluate patients following hospital discharge and provide timely assessment, treatment, or specialist referrals as needed

• Focus on clinical evaluation and care coordination with limited diagnostic workups and prescribing responsibilities

Qualifications:

• Master's degree as a Physician Assistant

• Minimum of 2 years of current experience in Internal Medicine or Adult Primary Care

• Missouri Physician Assistant certification/licensure

• Strong assessment, clinical judgment, and patient management skills
