Service Coordinator – Community Programs

The Threshold Incorporated | Full-Time | 40 Hours/Week

Make a difference by helping individuals build independence, develop life skills, and connect with their community!

The Threshold is seeking an organized, compassionate, and motivated Service Coordinator to support our Community Programs. This position coordinates meaningful community activities, supports individuals with disabilities, and helps ensure our programs run smoothly.

What You'll Do

• Plan and coordinate community outings, recreational activities, and skill-building opportunities.
• Support individuals in developing independence, social skills, and daily living skills.
• Provide guidance and support to direct service staff.
• Maintain client documentation, progress reports, and individualized support strategies.
• Collaborate with families, staff, and community partners.

What We're Looking For

• Associate degree and two years of related experience, or an equivalent combination of education and experience.
• Strong organizational, communication, and problem-solving skills.
• Ability to coordinate multiple activities and work collaboratively with others
• Valid Wisconsin driver's license, good driving record, and access to an adequately insured vehicle.
