Position: PT – Home Health
Facility: Providence at Home with Compassus Home Health, North Coast
Location: Seaside, OR and surrounding areas
Contract: 13 Weeks
Shift: Days, 8:00 AM – 4:30 PM
Schedule: 5x8s
Requirements

Minimum 1 year of skilled home health experience

BLS/BCLS required

Active Oregon PT license required at submission; pending license acceptance is noted in the job overview, so confirm eligibility

Home Care Home Base (HCHB) proficiency required

OASIS experience required for clinicians completing Start of Care (SOC)
