PHYSICAL THERAPIST (HOME HEALTH) – MOUNT ANGEL, OR
Position: PT – Home Health
Facility: Providence at Home with Compassus Home Health, Mid-Willamette Valley
Location: Mount Angel, OR and surrounding areas (Woodburn, Aurora, Molalla, Gervais, Silverton)
Contract: 13 Weeks
Shift: Days, 8:30 AM – 5:00 PM
Schedule: 5x8s
Requirements

Minimum 1 year of skilled home health experience

BLS/BCLS required

Active Oregon PT license required at submission

Home Care Home Base (HCHB) proficiency required

OASIS experience required for clinicians completing Start of Care (SOC)

No first-time travelers

Active driver's license and personal transportation required
