Licensed Practical Nurse (LPN)

True Kare Home Health Inc is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team in Palm Beach Gardens, FL. Responsibilities:

• Provide skilled nursing care to home health patients in accordance with physician-approved care plans
• Monitor and document patient health status, vital signs, and progress toward care goals
• Administer medications, wound care, and other treatments as prescribed
• Educate patients and family members on disease management, medications, and home care techniques
• Collaborate with RNs, physicians, and interdisciplinary care teams to ensure continuity of care
• Maintain accurate and timely clinical documentation in compliance with state and federal regulations
• Identify and report changes in patient condition to the supervising RN or physician promptly

Requirements:

• Active Florida LPN license in good standing
• Previous home health, long-term care, or clinical nursing experience preferred
• Strong knowledge of wound care, medication administration, and patient assessment
• Excellent communication and interpersonal skills with patients and care teams
• Reliable transportation and valid driver's license for home visits in the Palm Beach Gardens area
• CPR/BLS certification required
• Compassionate, self-motivated, and able to work independently in a home setting
