Dietitian

Position Description:

Provides nutritional services for the facility’s in-center and home patients, as applicable, to maximize patients’ nutritional status and improve clinical outcomes. Functions as a member of the interdisciplinary healthcare team, supporting FMCNA’s commitment to the Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) activities.

Actively participates in process improvement activities that enhance the likelihood of patients achieving their individualized, patient-specific goals as determined by their physicians.

Duties and Responsibilities:

Customer Service

• Supports and promotes the FMS culture through organizational values and customer service standards.
• Provides outstanding customer service to all internal and external customers.
• Develops and maintains effective relationships through timely and professional communication.
• Takes initiative to respond to, resolve, and follow up on customer service issues in a timely manner.

Principal Responsibilities and Duties:

• Assesses patients’ knowledge of diet and kidney disease and provides education appropriate to their learning needs regarding the relationship between diet, kidney disease, and treatment outcomes.
• Provides detailed, patient-specific education to patients, families, and caregivers regarding nutritional status, adequacy measures and results, fluid status, and nutrition-related laboratory parameters associated with their disease process.
• Completes nutritional assessments and participates in interdisciplinary comprehensive assessments.
• Collaborates with the interdisciplinary team to develop written, individualized comprehensive plans of care that address patients’ needs in accordance with established federal and applicable state licensure guidelines.
• Calculates diet prescriptions according to established standards of practice.
• Completes progress notes for all chronic dialysis patients according to FMCNA guidelines. Maintains complete and accurate medical record documentation in compliance with FMCNA policies and federal
