LVN Case Manager - Pathways Program

We are seeking a compassionate and patient-focused LVN Case Manager to support patients through an innovative Pathways Program. In this role, you will conduct outreach, enroll eligible patients, coordinate care, and help connect patients with the specialists and resources they need. The ideal candidate is an experienced LVN with case management experience who enjoys building relationships, improving patient outcomes, and closing gaps in care.

Responsibilities

• Conduct patient outreach and enroll eligible patients into the Pathways Program
• Complete approximately 25 meaningful patient conversations per day
• Spend 50-60% of the workday engaging with patients by phone
• Coordinate referrals and connect patients with appropriate specialists and community resources
• Identify and help close gaps in care through ongoing patient follow-up
• Educate patients on available services and resources
• Collaborate with providers, specialists, and the broader care team to ensure continuity of care
• Maintain accurate patient documentation and case notes
• Support patients throughout their healthcare journey and advocate for their needs
