We are seeking a compassionate and experienced Home Health Aid to join our team in Centralia, MO.

Responsibilities:

• Provide personal care and daily living assistance to our clients, including bathing, dressing, grooming, and toileting

• Assist with activities of daily living, such as meal preparation, medication management, and mobility support

• Monitor and report any changes in client condition or needs to the supervising nurse and/or physician

• Maintain a clean and safe living environment for clients

• Collaborate with other healthcare professionals to ensure comprehensive care

• Document all care activities and client interactions accurately and thoroughly

Requirements:

• Minimum of 2 years of experience as a Home Health Aid or related healthcare field

• Ability to lift 25 pounds and assist with transfers and ambulation

• Strong communication and interpersonal skills

• Ability to work independently and as part of a team

• Reliable transportation

• Compassion and empathy for clients and their families
