About the Role:

As an LPN at Landmark of Cicero, you will play a vital role in our interdisciplinary care team, working closely with our physicians, therapists, and other healthcare professionals to provide high-quality care to our residents. Your responsibilities will include:

• Providing direct resident care, including administering medications, wound care, and other treatments as ordered by the physician

• Monitoring resident conditions and reporting any changes or concerns to the care team

• Assisting with resident activities, such as bathing, dressing, and grooming

• Collaborating with the care team to develop and implement individualized care plans

• Maintaining accurate and detailed records of resident care and medications

• Participating in quality improvement initiatives to improve resident outcomes and care processes

Requirements:

• Current LPN license in the state of [State]
