Job Title: Licensed Practical Nurse (LPN)

Pay Rate:$35 - $40/hr

We are seeking a dedicated Licensed Practical Nurse to provide direct nursing care to residents in a skilled nursing and rehabilitation setting. The LPN will support residents’ daily healthcare needs, administer medications and treatments, monitor health conditions, and collaborate with registered nurses and other healthcare professionals to ensure quality care.

• Provide direct nursing care under the supervision of registered nurses and physicians, as appropriate.
• Administer medications and treatments according to physician orders and within the scope of practice.
• Monitor residents’ vital signs and report changes in their conditions.
• Participate in developing and implementing individualized care plans.
• Assist with coordinating resident care and transitions.
• Maintain accurate and detailed nursing records.
• Communicate with residents, families, and other healthcare professionals.
• Follow infection-control guidelines and patient-safety procedures.

• Current, valid Licensed Practical Nurse (LPN) license.
• BLS/CPR certification.
• Ability to provide safe and compassionate resident care.
• Strong observation, communication, and documentation skills.
• Ability to work collaboratively with nurses and the interdisciplinary care team.
• Previous LPN experience is beneficial; confirm any required minimum experience with the hiring team.
