Forte Human Capital is seeking a board-certified or board-eligible Family

Medicine or Internal Medicine physician to provide geriatric care to Veterans at the Fresno VA in Fresno, California.

This opportunity is ideal for a physician who values collaborative care and helping older adults maintain their health, independence, and quality of life.

Responsibilities

• Provide comprehensive evaluations and ongoing care for older adult Veterans.
• Manage acute and chronic medical conditions.
• Review medications and develop individualized treatment plans.
• Assess functional status, cognitive concerns, and preventive care needs.
• Coordinate care with specialists, nursing staff, and other members of the clinical team.
• Maintain accurate and timely documentation in the VA’s electronic health record.

Qualifications

• MD or DO degree.
• Completion of an accredited Family Medicine or Internal Medicine residency.
• Board certification or board eligibility in Family Medicine or Internal Medicine.
• Experience caring for geriatric patients preferred.
• Medical licensure and credentials meeting VA and assignment requirements.
• Strong communication skills and a commitment to patient-centered care.
