We are seeking LVNs interested in caring for patients requiring:
- G-Tube Care
- Tracheostomy Care (Trach)
- Ventilator Management
- Pediatric Nursing
- Medication Administration
- Chronic Disease Management
- Skilled Home Health Nursing
Experience with Trach, Ventilator, G-Tube, and Pediatric Patients is highly preferred but not required. Training opportunities are available for qualified candidates.
Responsibilities
As a Home Health LVN, you will:
- Provide skilled nursing care in accordance with physician orders
- Perform patient assessments and monitor changes in condition
- Administer medications and treatments
- Provide tracheostomy, ventilator, and enteral feeding care
- Document patient care accurately and timely
- Communicate with physicians, families, and clinical teams
- Educate caregivers and family members
- Support safe, compassionate, patient-centered care in the home
Ideal Candidates
- Active California LVN License
- Current CPR/BLS Certification
- Strong communication and clinical assessment skills
- Passion for patient advocacy and quality care
- Home health, pediatric, trach, vent, or G-tube experience preferred
