About the Role:

As a Home Health Aide, you will assist patients with daily living activities, mobility, and personal care needs. You will work under the supervision of a registered nurse or therapist to ensure that each patient receives tailored care that meets their unique requirements. Your dedication and empathy will make a significant impact on the lives of our patients and their families.

Responsibilities:

• Provide personal care assistance, including bathing, dressing, grooming, and toileting
• Assist with activities of daily living (ADLs), such as feeding, transferring, and ambulating
• Help patients with medication management, as needed
• Maintain accurate records of patient care and progress
• Collaborate with the interdisciplinary care team to develop and implement individualized care plans
• Support patients with chronic conditions, such as diabetes, hypertension, and arthritis

Requirements:

• High school diploma or equivalent required
• Completion of a state-approved Home Health Aide training program
• Current certification as a Home Health Aide or willingness to obtain
• Ability to lift and transfer patients, if necessary

• Compassionate and patient-centered approach
