Position Title: LVN / LPN – Behavioral Health
Location: Newark, NY 14513
Duration: 13 weeks
Work Schedule: Mid 5x8-Hour (14:00 - 22:00)
Pay rate: $30.00 - $43.00 /hr

Responsibilities for this position include, but are not limited to:

• Provide nursing care to residents in accordance with established care plans and facility policies.
• Administer medications and monitor residents' responses to treatment.
• Monitor and document vital signs, intake/output, and changes in residents' conditions.
• Implement behavioral management and de-escalation techniques.
• Assist with managing residents who may exhibit verbal or physical aggression.
• Maintain patient safety and follow protocols for a secured behavioral health unit.
• Collaborate with RNs, physicians, and other healthcare professionals to support resident care.
• Support residents with activities of daily living and personal care as needed.
• Maintain accurate documentation using EPIC.
• Assist with discharge planning and help residents transition to the least restrictive appropriate living environment.
• Follow infection control procedures and maintain patient confidentiality.

• Requirements:

• Active New York LPN license in hand at the time of submission.
• Minimum 2 years of nursing experience.
• Previous behavioral health, dementia care, or long-term care experience preferred.
• EPIC experience is mandatory.
• Current BLS certification.
• Ability to manage challenging behaviors and follow de-escalation protocols.
• Must be willing to float based on facility needs.
• Every other weekend availability required.
