Direct Care Staff / Caregiver – Full-Care (APD Eligible Required)

About the Position

Breakthroughs LLC is seeking a reliable, compassionate, and experienced female Direct Care Staff member to provide hands on support to an adult male with developmental disabilities in a family home setting in Wesley Chapel.

This is a full-care position requiring someone who is comfortable assisting with daily living activities and providing attentive, individualized support. The individual requires assistance with personal care and G-tube care, so we are looking for a caregiver who takes their responsibilities seriously, follows care instructions carefully, and prioritizes the individual's safety, dignity, and well-being.

The ideal candidate is patient, observant, dependable, and comfortable working closely with the individual and his family in a home environment. We are looking for someone who understands that direct care is more than simply being present it requires consistency, attentiveness, and a genuine commitment to providing quality care.

Schedule and Location

• Days: Monday through Friday
• Hours: 3:00 PM–11:00 PM
• Location: Wesley Chapel, Florida
• Setting: Private family home
• Consistency: Must be available for the full scheduled shift, five days per week.
  

Candidates must be able to consistently arrive on time and maintain this schedule. This position is not suitable for someone who needs a frequently changing schedule or cannot commit to weekday evenings.

Job Responsibilities

Responsibilities may include, but are not limited to:

• Providing hands-on assistance with activities of daily living (ADLs).
• Assisting with personal care, hygiene, grooming, dressing, and other care needs as required by the individual's support plan.
• Providing G-tube-related care in accordance with the individual's established care instructions, applicable training, and agency requirements.
• Monitoring the individual's comfort and well-being and communicating any concerns or changes in condition appropriately.
• Following the individual's support plan, health and safety procedures, and all applicable agency policies.
• Maintaining a safe, clean, and respectful home environment related to the individual's care.
• Supporting the individual's dignity, comfort, and quality of life.
• Communicating professionally with the family and agency regarding care-related needs, concerns, and incidents.
• Completing required documentation and service notes accurately and on time.
• Following all applicable confidentiality, HIPAA, and incident-reporting requirements.
• Remaining attentive and engaged throughout the entire shift.

Important: This is a hands-on caregiving role. Applicants must be comfortable providing full-care assistance and must not be seeking a position that consists solely of companionship or supervision.

Required Qualifications

To be considered, applicants must meet the following requirements:

1. APD Eligibility

• Must meet the applicable eligibility, screening, and onboarding requirements to work as direct care staff in an Agency for Persons with Disabilities (APD) service setting.
• Must be able to complete all required background screening, documentation, and agency onboarding procedures.
• Must be willing to complete any outstanding APD-required training and qualifications before providing services, as applicable.

2. Previous Caregiving Experience

• Previous direct care, caregiving, home health, disability support, or comparable hands-on care experience is required.
• Must have experience assisting individuals with personal care and daily living needs.
• Must demonstrate sound judgment, patience, professionalism, and the ability to follow detailed care instructions.
• Experience with G-tube care is strongly preferred. Applicants without this experience must be willing and able to complete the appropriate training and demonstrate competency before performing the relevant tasks.

3. Reliable Transportation — Mandatory

• Must have reliable transportation to and from Wesley Chapel for every scheduled shift.
• Must possess a valid driver's license, current vehicle registration, and current automobile insurance.
• Current car insurance and vehicle registration are mandatory requirements for this position.
• Applicants who cannot meet these transportation requirements will not be considered.

4. Schedule Availability

• Must be consistently available Monday through Friday, 3:00 PM–11:00 PM.
• Must be punctual, dependable, and able to commit to the assigned schedule.
• Must communicate promptly regarding emergencies, absences, or scheduling concerns.

5. Comfort Working in a Home With Animals

• Must be comfortable working in a private home where animals are present.
• Must be able to provide care in this environment without animal related concerns interfering with job responsibilities.
