Job Description

Physical Therapy Assistant (PTA) – Home Health (Ventura County)

Job Overview

Join our dynamic healthcare team at Vital Therapy Network as a Licensed Physical Therapist Assistant (PTA) dedicated to transforming patient care through compassionate, evidence-based practices. In this field-based role, you will deliver specialized, home-health physical therapy interventions under the supervision of a licensed Physical Therapist (PT). You will empower individuals of all ages across Ventura County to achieve their highest potential, promoting recovery, functional mobility, and independence for patients recovering from orthopedic injuries, neurological events, and complex chronic conditions.

 Key Responsibilities & Duties

  • Implement Therapeutic Interventions: Deliver skilled physical therapy treatments—including therapeutic exercises, gait training, balance interventions, and functional mobility modifications—aligned strictly with the PT's established care plan.
  • Foster Functional Independence: Assist and train patients with mobility-related Activities of Daily Living (ADLs) to maximize safe, independent living within their home environments.
  • Clinical Observation & Progression: Monitor patient responses during home visits; systematically track physical progress and clinical milestones, modifying treatment techniques within the care plan scope.
  • PT Collaboration: Coordinate closely with the supervising Physical Therapist to report progress, communicate status changes, and contribute to seamless discharge planning.
  • EHR Documentation: Complete accurate, timely progress notes using electronic health records (EHR) systems while adhering to strict medical terminology, home-health compliance, and California practice acts.
  • Diverse Population Care: Provide tailored interventions for diverse home health populations, including geriatrics, pediatrics, and individuals with neurodevelopmental disorders or complex neurodisabilities.
  • Patient & Family Coaching: Instruct patients, families, and caregivers on home exercise programs, safety protocols, and the proper use of adaptive mobility equipment.

 Required Skills

  • Anatomical Competency: Deep understanding of musculoskeletal anatomy, physiology, and kinesiology essential for implementing physical interventions safely.
  • Neurodevelopmental & Neurodisability Support: Proven ability or training to work effectively with individuals presenting with complex neurological needs (e.g., stroke recovery, Parkinson's disease, or Autism Spectrum Disorder).
  • Proficient Electronic Charting: Experience navigating point-of-care mobile electronic health records (EHR) software for precise field documentation.
  • Independent Time Management: Strong organizational skills to manage your daily travel schedule, patient appointment windows, and documentation timeline independently.

 Qualifications & Requirements

  • Licensure: Valid California license as a Physical Therapist Assistant (PTA) from the Physical Therapy Board of California.
  • Experience: Experience working with pediatric, adult, or geriatric populations in a clinical or home setting; home health experience is preferred but not required.
  • Logistics: Reliable personal vehicle, a valid driver's license, and proof of active auto insurance are mandatory. Must be willing to travel to patient residences throughout Ventura County.
  • Certifications: Current Basic Life Support (BLS) / CPR certification.
