Licensed Practical Nurse LTC

The Licensed Practical Nurse (LPN) will provide compassionate, comprehensive care to general long-term care residents. Reporting to the Director of Nursing, this role supports resident well-being through clinical care, medication administration, care coordination, safety monitoring, and effective communication with residents, families, and the care team.

12 Week Contract with the option to extend

Responsibilities

• Provide resident care to support comfort, dignity, and well-being.
• Administer medications safely and accurately.
• Monitor and document resident vital signs.
• Contribute to individualized care planning and care coordination.
• Provide wound care and monitor healing progress.
• Complete timely and accurate clinical documentation.
• Assist residents with mobility and activities of daily living.
• Monitor resident safety and communicate changes in condition.
• Maintain effective communication with families and the interdisciplinary care team.

Qualifications

• 1+ years of experience in long-term care nursing.
• Practical Nursing Diploma.
• Licensed Practical Nurse (LPN) license.
• Basic Life Support (BLS) certification.
• Experience with medication administration and geriatric nursing.
• Experience using Electronic Health Records.
• Knowledge of wound care and infection control.
• Skills in patient assessment and clinical documentation.
• Strong communication and teamwork skills.
