Responsibilities:

• Monitory admissions, discharges and transitions of care for CCBHC clients and potential clients
• Participate in hospital admission/discharge planning to coordinate behavioral health services as part of discharge plan
• Communicate with community partners in order to collaborate and assist in coordinating services
• Facilitate travel/transportation to assist clients in accessing necessary services
• Accurately complete all required documentation
• Participate in agency training and ongoing professional development as required
• Assist with Center intake process as needed and appropriate
• Provide Targeted Case Management (TCM) to clients as needed and appropriate
• Other duties as assigned.

Requirements:

• Bachelors degree in Psychology, Counseling, Social Work or related area; or 4 years of equivalent work experience.
• Complete Case Management Live training within 6 months of employment
• Background void of any issues related to abuse or neglect.
• Valid Kansas driver's license with favorable driving record

Desired Qualifications:

• Working knowledge of social service resources
• Knowledge of general recovery concepts
• Experience with partnership development
• Experience motivating/advocating for persons with disabilities
• Bilingual in Spanish
