Certified HHA or PCA - Otsego, Delaware, Schoharie Co.

Role and Responsibilities: Provides personal care and supportive services to individuals in the community, in the privacy of their home. 

Essential Functions:

• Provides health-related services as outlined in the plan of care, authorized by a physician, and established by a registered nurse or licensed therapist, under the direction of the HHA/PCA matrix established by the NYS Department of Health. 
• Provides assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home. 
• Immediately reports changes in client’s condition to supervisor. 
• Ensures the safety of the client, client’s family and/or the employee. 
• Maintains HIPAA.  
• Completes and submits necessary client documentation including EMR charting tasks and observations on Android device. 
• Attends ongoing in-service programs in a timely manner, to maintain certification. 
• All other duties as assigned.
