Community Care Youth Case Manager  

BH Respite Provider – Coshocton, OH 

Part-time Position 

Quality Moments is looking for a reliable, enthusiastic, and supportive individual to work directly with youth and their families by providing short-term, temporary relief for caregivers through facilitating services focused on meaningful interactions with youth.

This position offers an excellent opportunity to provide youth with individualized skill-building opportunities within home and/or community-based settings and offers scheduling flexibility to allow the needs of the youth, caregiver, and provider to be met.   

 Qualifications and Responsibilities 

·         Must be at least 21 years of age.

·         Diploma or GED required.

·         Valid driver’s license and reliable transportation with current automobile insurance meeting state minimum liability requirements.

·         Must be able to work evenings and weekends.

·         Prior experience in case management, social work, counseling, human services, or a related field helpful; minimum three years’ experience preferred but not required.

·         Is dependable, professional, and passionate about helping youth succeed in their community.
