Leeds Professional Resources is seeking a compassionate, organized Intake Specialist to help older adults and individuals with disabilities navigate Medicaid long-term care services. This opportunity is ideal for someone who enjoys helping others, explaining complex information clearly, and keeping important details on track.

Responsibilities

• Conduct intake interviews and standardized screenings to assess potential eligibility for Medicaid long-term care services.
• Explain program requirements, application steps, and required documentation to individuals and families.
• Assist applicants with gathering information and completing eligibility paperwork.
• Maintain accurate electronic records, screening results, and application status updates.
• Track pending applications and follow up on missing information.
• Coordinate with state agencies, healthcare professionals, and community resources to support the eligibility process.
• Connect individuals with additional services and resources as appropriate.
• Protect confidential information and provide patient, respectful service.

Qualifications

• Bachelor’s degree; or associate’s degree with two years of related experience; or high school diploma/GED with four years of related experience.
• Strong communication skills and a professional telephone manner.
• Excellent organization, attention to detail, and follow-through.
• Working knowledge of Microsoft Office and electronic recordkeeping.
• Ability to manage multiple priorities and communicate with individuals facing challenging circumstances.
• Successful completion of required background screening.

Experience in patient intake, healthcare administration, financial counseling, benefits eligibility, or social services is especially relevant.
