School-Based Social Worker - Des Plaines, Illinois

Make an immediate impact supporting elementary students as a contract Social Worker in a diverse learning environment. This full-time opportunity runs from February 3, 2027, through April 14, 2027, and involves working with 3rd to 5th-grade students at the elementary level. The role focuses on providing vital support to students with IEPs, 504 plans, and a range of learning needs during a staff leave. Key Qualifications:

• Active Illinois PEL with School Social Worker endorsement required
• Experience working with students in grades 3-5, particularly those with IEPs and 504 Plans
• Familiarity with Embrace for IEP management and MyServiceTracker for Medicaid billing is highly preferred
• Strong organizational and communication abilities
• Commitment to student advocacy, inclusion, and mental health
• Prior experience supporting general education, instructional, and structured classrooms is a plus

Role Responsibilities:

• Deliver social work services to a caseload of 12 general education, 6 instructional, and 3 structured students with IEPs, as well as 11 students with 504 Plans
• Collaborate closely with educators, support staff, and families to address individual student goals
• Participate in IEP meetings, write reports, and monitor progress
• Implement evidence-based interventions to foster emotional and academic growth
• Maintain accurate records and ensure compliance with district, state, and federal mandates
• Utilize Embrace and MyServiceTracker for documentation and billing purposes
