Are you an organized and detail-oriented nurse who enjoys coordinating resident care and making a difference in long-term care? Hallmark Healthcare of Pekin is looking for a dedicated MDS Coordinator (RN or LPN) to join our team!

This is a great opportunity for a nursing professional who enjoys working collaboratively, maintaining accurate clinical documentation, and helping ensure residents receive individualized, high-quality care.

What You'll Do:

• Coordinate and complete MDS assessments in accordance with applicable guidelines and deadlines.

• Ensure accurate and timely completion of resident assessments and care plans.

• Collaborate with nursing, therapy, dietary, social services, and other departments.

• Participate in care plan meetings and interdisciplinary team discussions.

• Monitor assessment schedules and ensure documentation is complete and accurate.

• Support compliance with state and federal regulations.

• Help identify opportunities to improve resident outcomes and quality of care.

What We're Looking For:

• Current Illinois RN or LPN license required.

• Previous MDS experience preferred.

• Knowledge of MDS assessments, care planning, and long-term care regulations.

• Strong organizational and time-management skills.

• Excellent communication and teamwork abilities.

• Attention to detail and ability to meet deadlines.
