MDS Coordinator (LPN/RN)

Pay

LPN: $38.00 - $40.00 per hour
RN: $83,000 - $93,000 annually

Position Summary

The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI)/MDS process to ensure accurate, timely, and compliant assessments. This role works closely with the interdisciplinary team to support quality resident care, reimbursement, and regulatory compliance.

Qualifications

• Current, active LPN or RN license in the Commonwealth of Virginia

• Minimum 1 year of MDS experience required

• Experience in a skilled nursing or long-term care setting preferred

• Strong knowledge of MDS 3.0, RAI process, Medicare, Medicaid, and PDPM

• Excellent assessment, documentation, and communication skills

• Ability to work collaboratively with an interdisciplinary team

Responsibilities

• Complete accurate and timely MDS assessments in accordance with federal and state regulations

• Coordinate the Resident Assessment Instrument (RAI) process

• Ensure compliance with CMS guidelines and reimbursement requirements

• Collaborate with nursing, therapy, social services, dietary, and other departments to develop comprehensive care plans

• Monitor assessment schedules and ensure documentation deadlines are met

• Review medical records for accuracy and completeness

• Participate in quality assurance and performance improvement initiatives

• Stay current with regulatory changes affecting MDS and reimbursement
