Hospice Director of Patient Care Services - Chicopee, MA

We are hiring an experienced Hospice Director of Patient Care Services for a growing hospice program serving patients and families in Western Massachusetts. This is a full-time leadership role with real impact.

Responsibilities

• Direct the clinical team of RNs, LPNs, aides, social workers and chaplains

• Ensure quality, compliance and survey readiness under Medicare hospice CoPs

• Oversee admissions, plans of care, IDG meetings and clinical documentation

• Manage staffing, on-call coverage, productivity and budgets

• Recruit, train and mentor clinical staff and lead QAPI

Qualifications

• Active Massachusetts RN license (BSN preferred)

• 3+ years of hospice or home health clinical leadership

• Strong knowledge of Medicare hospice Conditions of Participation

• Experience with EMR systems and survey preparation

• Compassionate, organized and team-focused leader

Schedule: Full-time, on-site

Compensation: $120,000 - $145,000 per year + benefits
