We are seeking a dedicated Registered Nurse (RN) to provide skilled nursing care throughout our South/Southwest territories. This role includes routine skilled visits, OASIS assessments, care coordination, and patient education. RNs work independently in the field while supported by responsive clinical leadership and a coordinated office team.

• Provide skilled nursing visits including wound care, medication management, post‑acute care, and disease‑specific interventions.

• Complete OASIS assessments (SOC, ROC, Recertification, Discharge) within CMS and agency timelines.

• Develop and update individualized plans of care in collaboration with physicians and interdisciplinary team members.

• Assess home environments for safety risks, fall hazards, and medication concerns; document and address findings.

• Educate patients and caregivers on disease processes, medications, symptom recognition, and emergency criteria.

• Communicate changes in condition promptly and obtain updated orders as needed.

• Document all care accurately and timely in the EMR.

• Coordinate care with PT/OT/ST, social work, and home health aides.

• Manage a structured full‑time caseload within assigned South/Southwest territories.

• Participate in case conferences and quality improvement activities.

Required:

• Active Registered Nurse (RN) license in Illinois.

• Minimum 1 year of nursing experience (home health preferred).

• Strong clinical judgment and ability to work independently.

• Reliable transportation and valid driver's license.

• Proficiency with electronic documentation Well Sky Kinnser
