- Support inpatient heart failure patients by identifying care needs, coordinating transfers, and facilitating referrals to the heart failure clinic
- Conduct post-discharge follow-up calls within 72 hours and help ensure follow-up appointments are scheduled within seven days
- Communicate with Advanced Practice Nurses, Cardiologists, and other clinical team members to ensure continuity of care
- Prepare patients for appointments and procedures while maintaining efficient clinic flow
- Maintain clinical equipment, crash carts, and refrigerator quality control
- Respond to patient calls, assist with general nursing needs, and support daily clinic operations
- Initiate ACLS interventions when necessary and maintain compliance with infection control and patient safety standards
- Maintain required certifications and stay current with clinical standards and best practices
Qualifications:
- Active Registered Nurse (RN) license
- Hands-on experience working one-on-one with patients experiencing heart failure
- Experience with pulmonary hypertension (PH) strongly preferred
- Familiarity with 6-minute walk tests and cardiac patient assessments
- Cardiac experience in a cardiac group preferred, followed by inpatient or outpatient clinic experience
- Experience with ECG/EKGs, Holter monitors, event monitors, and cardiovascular devices preferred
- Intermediate computer skills; Epic experience is a plus
- Strong organizational, time management, and task management skills
- Ability to work autonomously while collaborating with physicians and clinical teams
- Strong patient education and care coordination skills
