Position Overview

Proxena Health Group is recruiting a Board-Certified or Board-Eligible Family Medicine Physician for a permanent opportunity in Pueblo, Colorado.

Key Responsibilities

• Provide full-spectrum family medicine and preventive healthcare.
• Build and maintain meaningful, long-term relationships with patients and families.
• Diagnose and manage acute and chronic medical conditions.
• Support preventive health, wellness and patient education.
• Collaborate with physicians and advanced practice providers to coordinate patient care.
• Participate in an after-hours call rotation with Family Medicine colleagues.
• Contribute to clinical quality initiatives and continuous improvement.
• Coordinate access to laboratory, imaging, specialty and other healthcare services.

Required Qualifications

• Board-Certified in Family Medicine or genuinely Board-Eligible.
• Board-Eligible candidates must be within seven years of completing residency.
• Completion of an appropriate Family Medicine residency.
• Active Colorado medical license or eligibility to obtain one.
• Most recent NPDB Self-Query Report, issued within the preceding 30 days. This is mandatory at presentation.
• DEA registration must be obtained before starting employment.
• Candidates must disclose whether they require J-1 or H-1B visa support during submission.

Preferred Qualifications

• Active Colorado medical license.
• Clean malpractice and background history.
