Position Overview

Proxena Health Group is seeking a qualified Physical Therapist for a contract opportunity in San Fernando, California.

The successful candidate will provide evidence-based therapy services, collaborate with healthcare professionals and support patients in achieving improved functional outcomes.

Key Responsibilities

• Provide safe and effective physical therapy services within professional scope of practice.
• Assess patients and support appropriate treatment interventions.
• Develop and implement patient-centered therapy plans.
• Monitor patient progress and document treatment outcomes.
• Collaborate with physicians and interdisciplinary healthcare professionals.
• Maintain accurate clinical documentation.
• Follow applicable facility policies, professional standards and infection-control procedures.

Qualifications

• Active, unrestricted Physical Therapist license appropriate for practice in California.
• BLS certification.
• One to two years of recent relevant clinical experience preferred.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and collaboratively.
