Position Overview

Proxena Health Group is recruiting a qualified Licensed Practical Nurse for a temporary Home Health opportunity in Pittsfield, Massachusetts.

The successful candidate will provide nursing care in patients' homes, collaborate with healthcare providers and community representatives, and help patients maintain, restore and improve their health.

This position requires the ability to work independently, exercise sound clinical judgment and manage home-health responsibilities.

Key Responsibilities

• Provide nursing interventions to promote, maintain and restore patient health.
• Deliver appropriate nursing care and rehabilitation support in patients' homes.
• Coordinate care with other healthcare professionals and community representatives.
• Evaluate patients' and families' responses to nursing interventions.
• Maintain accurate clinical records and communicate patient information appropriately.
• Apply sound clinical judgment and critical-thinking skills.
• Manage assigned duties independently while following established clinical guidelines.
• Accept on-call responsibilities when required.
• Maintain flexibility regarding work hours and patient assignments.

Required Qualifications

• At least one year of acute nursing experience.
• Previous home-health nursing experience.
• Active Massachusetts LPN license in hand at submission. Pending licenses will not be accepted.
• Current driver's license and automobile insurance.
• Personal vehicle is mandatory.
• Ability to drive in adverse weather conditions.
• Ability to work independently with minimal supervision.

Preferred Qualifications

• Certified Home Health experience, including OASIS proficiency.
• Infusion therapy experience.
• Knowledge of home-health reimbursement and regulatory requirements.
• Experience managing and scheduling an independent patient caseload.

Important Assignment Conditions

• Travelers must have temporary accommodation within 50 miles of the facility and preferably a commute of no more than 45–60 minutes.
• Candidates whose permanent residence is within 50 miles of the facility will not be considered.
• Travelers must have their own vehicle.
• Candidates must provide their permanent address in their submission profile.
• All requested time off must be declared at submission. Scheduling requests made after an offer or interview will not be accepted.
• Candidates must be available to work two holiday coverage blocks when scheduled or needed, involving the Thanksgiving, Christmas and New Year holiday periods.
