Responsibilities:

• Administer medications, treatments, and therapies as prescribed by physicians

• Monitor and report changes in residents' conditions, including vital signs and neurological status

• Develop and implement individualized care plans in collaboration with the interdisciplinary care team

• Provide education and support to residents and families on healthcare issues and treatment options

• Collaborate with other healthcare professionals to ensure seamless transitions and continuous care

• Participate in Quality Improvement initiatives to enhance resident outcomes and overall care

• Demonstrate a commitment to excellent customer service and respect for the dignity and autonomy of each resident
