Job Title: Licensed Practical Nurse – Long-Term Care

Schedule: Mix of 8-, 12-, and 16-Hour Shifts, Approximately 3–5 Working Days per Week
Guaranteed Hours: 36 Hours per Week
Weekend Requirement: Primarily Weekends

Pay Rate
Local: $35.00–$37.00/hr
Traveler: $36.00–$38.00/hr

Required Qualifications

• Active Licensed Practical Nurse license.

• Flexibility to work variable day and evening shifts.

• Availability to work primarily weekends.

• Ability to work 8-hour, 12-hour, and 16-hour shifts.

• Willingness to work additional hours and overtime when needed.

• Strong clinical, communication, and documentation skills.

Preferred Qualifications

• Previous experience in Long-Term Care or skilled nursing.

Responsibilities

• Administer medications and treatments according to physician orders and facility protocols.

• Monitor residents' health conditions and report changes to the appropriate healthcare provider.

• Perform nursing assessments and document residents' conditions.

• Coordinate resident care with Certified Nursing Assistants and other healthcare team members.

• Assist with care planning and implementation.

• Provide wound care and other nursing interventions as assigned.

• Maintain accurate clinical documentation.

• Promote resident safety, comfort, and dignity.

• Follow infection-control standards and facility policies.
