Social Services Assistant

ALIYA on 87th is seeking a compassionate and organized Social Services Assistant to support the Social Services department in meeting the psychosocial needs of our residents. The ideal candidate will have strong communication and interpersonal skills, a genuine commitment to resident well-being, and the ability to work collaboratively with residents, families, and facility staff.

Key Responsibilities

• Assist with the planning, coordination, and implementation of the facility's social services programs.
• Support residents and families with adjustment to the facility, including admissions, transfers, and discharges.
• Assist residents with identifying and addressing social, emotional, and psychological needs.
• Maintain accurate and timely documentation, including resident assessments, care plans, and progress notes.
• Participate in care plan meetings and collaborate with the interdisciplinary team to support resident needs.
• Serve as a resource for residents and families, addressing concerns and helping resolve issues.
• Assist with discharge planning and coordinate with community resources and outside agencies.
• Advocate for residents' rights, dignity, and personal preferences.
• Assist with identifying residents who may require additional social services support.
• Ensure social services documentation and practices comply with applicable federal and state regulations.

Qualifications

• Bachelor's degree in Social Work or a related field preferred.
• Previous experience in social services, preferably in a skilled nursing facility or healthcare setting.
• Knowledge of long-term care regulations and resident rights preferred.
• Strong communication, organization, and interpersonal skills.
• Ability to work effectively with residents, families, and interdisciplinary team members.
