Wound Care Coordinator

ALIYA on 87th is seeking an experienced and dedicated Wound Care Coordinator to oversee and coordinate the facility's wound care program. The ideal candidate will have strong clinical knowledge, attention to detail, and the ability to work collaboratively with nursing staff and interdisciplinary team members to ensure residents receive appropriate wound prevention and treatment.

Key Responsibilities

• Oversee and coordinate the facility's wound care program, ensuring residents receive timely and appropriate wound assessments and treatments.
• Perform and monitor wound assessments, documenting wound status, measurements, progress, and treatment plans.
• Develop, implement, and update individualized wound care plans in collaboration with physicians, nursing staff, and other healthcare professionals.
• Monitor residents for changes in skin condition and ensure appropriate interventions are implemented.
• Provide guidance, education, and support to nursing staff on wound prevention, treatment protocols, and proper documentation.
• Ensure wound care treatments are completed as ordered and accurately documented.
• Monitor wound healing progress and communicate concerns or changes to physicians and the Director of Nursing.
• Coordinate with the interdisciplinary team to identify residents at risk for skin breakdown and implement preventive measures.
• Maintain accurate wound care records and ensure compliance with applicable federal and state regulations.
• Participate in clinical meetings, audits, and quality improvement initiatives related to wound care.

Qualifications

• Current Illinois RN or LPN license required.
• Previous wound care experience in a skilled nursing facility or healthcare setting required.
• Wound care certification preferred.
• Strong knowledge of wound assessment, treatment, prevention, and infection control practices.
• Familiarity with skilled nursing facility regulations and clinical documentation requirements.
• Excellent communication, organizational, and clinical assessment skills.
• Ability to educate and support nursing staff while managing multiple clinical priorities.
