As a part of our continued growth, we are searching for a new Infertility Case Manager to join our Reproductive Resources Team.
As an Infertility Case Manager you will be responsible for clinical operations and medical management activities across the continuum of care for members seeking Infertility treatment services.
Responsibilities include:
- Making outbound calls to assess members' health status
- Identifying gaps or barriers in treatment plans and coordinate ongoing care with providers
- Providing patient education to assist with self management and healthy lifestyle changes
- Documenting and tracking progress from initial assessment through post-operative care
Requirements
Requirements:
- Current, unrestricted RN license in your state of residence
- 3 or more years of nursing experience in Infertility
- Computer proficiency including the ability to type and talk at the same time and to navigate within Microsoft Windows
Preferred Qualifications:
- Previous Case Management experience
- Certified Case Manager (CCM)
- Compact state RN license
- Managed Care experience
- BSN
