Now is the perfect time to make the change from direct patient care to a telephonic case manager role!! We are adding a highly skilled Oncology RN to their growing team. As an Oncology Case Manager, you will serve as a comprehensive telephonic case manager to oncology patients who have been referred to our program.
Key responsibilities of the Oncology Case Manager include:
- Providing benefits information and medical care coordination
- Gathering diagnosis information from patients, as well as treatment and medical history
- Collaborating with contract providers, vendors, and inpatient/outpatient services to coordinate care
- Identifying methods for cost savings while providing quality service
- Educating and empowering the consumer to take an active role in the decision making process related to their treatment options.
- Supporting and educating the member through the treatment process as well as management of the side effects of their diagnosis and treatment
Requirements
Requirements:
- Current and unrestricted RN license in the state of FL
- 3+ years clinical experience in a hospital setting, acute care and/or direct care setting post licensure
- 3+ years Oncology experience in a hospital, outpatient, home care and/or hospice setting
- Computer proficiency to include comfort with MS Windows programs (Word, Excel, Outlook) and solid typing skills
Additional Assets:
- Case management and/or care coordination experience
- Case management certification (CCM)
- OCN certification HIGHLY preferred
- FL Medicaid experience
- Discharge planning experience
- Bilingual English/Spanish
