We are seeking a full time, team-oriented, certified Nurse Practitioner (NP) to add to our Family Medicine team. St. Joseph's Family Medicine is proud to be a NCQA, level III patient and family centered medical home. We provide comprehensive, compassionate care that is centered around each individual patient, their language and culture. The medical home allows for a continuum of care for patients designed to help them control their current health issues, help prevent future issues, and deter them from using our emergency room as their primary access to care. You will provide this care in a modern, well- equipped office that is adjacent to St. Joseph's Hospital and Medical Center. A fully integrated electronic medical record and billing system facilitates our providers in offering a very high level of excellence in the care of their patients.
The Family Medicine Center is located on the hospital campus in an attractive and recently renovated building just steps away from the main hospital tower. The Center is over 20,000 sq feet and sees approximately 1,800 patient visits per month. Because of the quality of care and scope of services, we were the first Family Medicine Center to attain NCQA Level 3 Patient Centered Medical Home status in 2011.
