Providence is calling a Social Worker to Providence Sacred Heart Medical Center in Spokane, WA.
This is a part time (.80 FTE) position 64 hrs every two weeks.
We are seeking a social worker to develop post-hospital plans to assure seamless transitions for patients and their families.
In this position you will:
Enable patients and their families to recover or maintain optimal levels of physical, mental, and psychosocial well being.
Use your exceptional communication skills to help patients and families deal with the impact of illness, extended care placement, or end-of-life planning.
Provide psychosocial assessment, counseling, and discharge planning for long term care.
Implement social work treatment plans and documentation consistent with insurance guidelines.
Work collaboratively with other members of the patient-care team to ensure successful care management.
Required qualifications for this position include: Master's degree in Social Work. Applicants who apply for an Agency Affiliated Counselor credential through the Department of Health within seven days of employment can provide services within the agency for up to 60 days while the department processes their application. Applicants must stop working on the 60th day of employment if the credential has not been issued. During this time, applicants may not provide unsupervised counseling until the department of their employer has completed a criminal background check. Unsupervised means the supervisor is not physically present at the location where the counseling occurs OR possess a valid and unencumbered Washington State License/Registration for one of the following: Licensed Advance Social Worker or Licensed Independent Clinical Social Worker required.
Preferred qualifications for this position include:
Two (2) years hospital experience.
