Job Description:
The Coordinator of our Non-Medical Home Care Services is expected to perform a variety of duties that relate to client care including Quality Assurance visits with clients and family members. Continuously evaluating the care plan through a series of ongoing communications and visits will ensure high quality care, client satisfaction and retention as well as identifying additional service needs when appropriate to allow our clients to age in place.

Continuously evaluating the care plan through a series of ongoing communications and visits will ensure high quality care, client satisfaction and retention as well as identifying additional service needs when appropriate to allow our clients to age in place.
Job Requirements:
Master's Degree in Social Work or Bachelor's Degree with appropriate experience in home care or senior care
Knowledge of resources available to Elders and their families within the community supporting our clients' desire to age in place
Willingness to travel throughout the state to accomplish goals of Quality Assurance program
Ability to develop relationships with other providers and potential referral sources
