Registered Nurse Case Manager - Full-time / Part-time

Responsibilities:

• Provide home health services to individuals, and their families.
• Managing care for a caseload of primarily complex clients under general direction of management.
• Take a leadership role and collaborate with the multidisciplinary group and other caregivers to meet specific goals.
• Provide supervision to Home Health Aides, and clinical supervision of staff RN's.
• Assure access to other disciplines, as needed.

Requirements:

• Current RN license
• Minimum of an Associate’s Degree in nursing
• Basic computer skills
• Working knowledge of nursing principles, practices & policies
• Valid Utah Drivers License
• Home Care and Hospice experience preferred
