Senior Home Care Client Manager - Full-time

Home Instead Senior Care is looking for an organized individual to join us in our mission to enhance the lives of aging adults and their families.  The position of Client Care Coordinator performs a variety of duties that relate to client care including care consultations with potential clients and family members, client/CAREGiver introductions and quality assurance visits with existing clients.

Responsibilities

• Meet with potential clients and family members to discuss their needs and provide solutions in the form of a service plan
• Work with other team members to coordinate various aspects of a client's care
• Create and maintain client records
• Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients
• Demonstrate open and effective communication with clients, family members, the franchise owner, colleagues and CAREGivers, and referral providers/care providers

Qualifications

• Possess excellent verbal and written communication skills and the ability to listen effectively
• Be service oriented and able to work well in a team environment in a fast-paced organization
• Proficient with computer and various software programs
• College degree preferred
• Experience in home care, health care or senior-related industry preferred
