Job Summary/Essential Duties\:
  • Assesses patient status with patient/family input and identifies age-appropriate patient care needs/problems that includes bio-physical, psycho-social, spiritual, environment, self-care, and educational needs and will serve to focus the provision of nursing care at admission, transfer, and discharge.
  • Plans individualized age appropriate patient care through a interdisciplinary team approach and through interdisciplinary patient care conference, ongoing patient assessments, re-evaluation of patient problems or needs.
  • Utilizes professional skills and judgement in prescribing, delegating and coordinating the plan of care; includes appropriate resources to manage patient care path/CareMaps/variances. Administers medications and treatments according to nursing policy.
  • Guides and directs professional and nonprofessional personnel in the delivery of patient care, evaluates patient care effectiveness, modifies the plan of care according to age appropriate patient nursing policy. Utilizes care paths/CareMaps to develop and modify the patient's plan of care. Monitors variances and takes action on these variances.
  • Provides the following nursing services\: a. Independent; b. Inter-dependent; c. Delegated.
  • Identifies patient/significant other learning needs and initiates and provides for age appropriate education.
  • Respond to patient calls, anticipates individual patient needs, and assures patient safety and comfort through regular rounds, environmental maintenance, and other appropriate safety measures.
  • Demonstrates the ability to perform in an accurate and precise manner in crisis and emergency situations. Demonstrates effective management of rapidly changing situations.
Hours\: 6\:30p-6\:30a. every other weekend, every other holiday.
Qualification:
QUALIFICATIONS\:
Experience
Required\:
One year mental health care related experience conducting group psychotherapy (requirement for Medicare approved Partial Hospital settings).
 
Education/Licensure
Graduate with a nursing degree from an NLN approved school of nursing preferred.
Must have a Current Registered Nurse License in Nebraska or Iowa. Nebraska and Iowa are compact states, so license must be held in the state of your residence.
Heritage Center prefers BSN.
 
Additional requirements include\:
Crisis Prevention Intervention CPI required within 6 months of hire
 
BLS or BLS instructor certification is required upon hire. Acceptable credentialing bodies and certifications include the following\: American Red Cross\: CPR/AED for the Professional Rescuer, American Safety and Health Institute\: CPR, PRO, must say “Professional Level-with AED”, and American Heart Association\: Basic Life Support for Healthcare Providers.
 
Knowledge/Skills/Abilities
Job requires continuous use of reading skills and verbal communication skills. Job requires frequent use of written communications skills. Job requires delegation, conflict resolution, individual and group interactions, time management, critical thinking and stress management. Incumbents must possess knowledge of how to provide care to a diverse population of patients and the ability to communicate and work with people from a variety of age-groups, social strata and ethnic backgrounds.
