LPN - Specialty Medication Case Manager

Looking for a LPN, licensed in the state of Florida, to work in a Case Management position with a Specialty Pharmacy in Orlando.

They would handle about 40 outbound calls a day. Review patient information and obtain missing clinical documentation, provide clinical support and administrative assistance.

There are three main responsibilities;

Call Counseling

• Call the patient, collect information and counsel the patients if they have any questions about the medication.

Clinical Escalation

• Receive an escalation from a Patient Care Coordinator, they would need to review the case to determine if a call needed to be made or if other steps needed to be taken.

Adversity Reports

• When calling patients, this person would collect all the side effects the person is experiencing and create reports to be sent into the drug manufacturer. They would record if the person is experiencing head aches, pain, etc.

• Review patient information and obtain missing clinical documentation from physician offices and insurance companies
• Interpret clinical information such as laboratory results and diagnosis codes represented in patient medical records
• Enter clinical information in internal and external databases
• Document all communication efforts with patient, physician and insurance company and relevant information into internal and external systems
• Verify insurance and benefits information for highly complex disease states in order to schedule medication delivery
• Conduct monthly counseling to patients and their family to ensure adherence to providers therapy plan
• Assist with insurance verification and patient care coordination
